RI ranks among worst states for doctors in 2026 WalletHub report

PROVIDENCE, R.I. — Rhode Island ranks among the worst states in the country for doctors in 2026, according to a new study from WalletHub.

The report places Rhode Island 49th out of 51, putting it near the bottom nationwide and among the most challenging places for physicians to practice. 

Among worst states in the country

Only a handful of states ranked lower.

Rhode Island was grouped with states like New York and New Jersey among the worst overall for doctors. 

The rankings are based on two main areas:

  • Opportunity and competition
  • Medical environment

WalletHub used 19 different metrics, including salary, job demand, malpractice costs and hospital quality.

High competition a major factor

One of Rhode Island’s biggest challenges is competition.

The state ranks among the highest in the nation for projected physician competition. 

That means more doctors are competing for fewer opportunities, making it harder to establish a practice.

Pay also ranks low

Physician pay is another issue.

When adjusted for cost of living, Rhode Island ranks near the bottom nationally for doctor wages.

Lower relative pay, combined with high living costs, makes the state less attractive compared to others.

Mixed picture for healthcare overall

Despite the poor ranking for doctors, Rhode Island performs better in other healthcare measures.

Previous WalletHub data has ranked the state among the top for overall healthcare access and quality. 

That contrast suggests strong patient care systems, but a difficult environment for providers.

Top states offer better conditions

States like Montana, Indiana and South Dakota ranked among the best places for doctors.

These states offer higher pay, less competition and more favorable working conditions.

Ongoing concern for Rhode Island

Rhode Island has ranked near the bottom in recent years.

The continued low ranking raises concerns about the state’s ability to attract and retain physicians.

Fewer doctors could lead to longer wait times and reduced access to care, especially in primary care.
